The following permission forms are available for download from this

team registrationf form is now available
and due back to Coach
Tepper by 9/26/2011.
Permission
form for North Bend Holiday Festival on Dec 10th
District policy now requires all
perspective volunteers to complete a new
Volunteer Disclosure Form. Please download, print, fill out,
and return it to the school with a copy of your driver's license. NOTE:
In the section where it asks for your children's teacher(s),
PLEASE ADD COACH TEPPER!
